By checking the "I Agree to the Terms & Conditions" checkbox on this page, I hereby consent to have my story (referred to as Posted Story) and personal likeness (referred to as Personal Images) used, released, published, copied, exhibited, moderated, and posted on the Internet by Centinel Spine, LLC and its affiliates and companies with which it collaborates (collectively, the “Centinel Parties”), and acknowledge that by checking the "I Agree to the Terms & Conditions" checkbox on this page, I agree to the following:

• I consent to the digital manipulation and/or editing of my Personal Images, including without restriction, changes or alterations as to color, size, shape, perspective, context, foreground, background, and/or order of appearance of in images.
• I consent to the editing of my Posted Story, as well as the removal of my Posted Story from any of Centinel Spine LLC's publishing platforms, in order to adhere to the rules and regulations of the markets in which its medical devices are marketed and in which Centinel Spine performs business—in addition to Centinel Spine's need to ensure the quality, legibility, and accuracy of the information presented on its publishing platforms.
• I understand that Centinel Spine, LLC has an obligation to ensure that all stories and/or images published on its platforms adhere to the rules and regulations of the markets in which its medical devices are marketed and in which Centinel Spine performs business.
• I understand that my consent is voluntary, that I am not required to agree to these Terms & Conditions, and that I may in fact refuse to agree to them, thereby prohibiting the Centinel Parties from obtaining or using any stories or images that include me.
• I understand that my Posted Story and Personal Images may include my name, age, health status, and other personal information and that the inclusion of my Posted Story and Personal Images in whatever medium deemed appropriate by the Centinel Parties may reveal sensitive health information about me, including health conditions.
• I release and discharge the Centinel Parties and their affiliates, officers, agents, and employees; and each and all persons involved in creating my Posted Story and Personal Images from any liability connected with the alteration, use, or publication of such materials.
• I waive all rights I may have to claims for payment or royalties in connection with any exhibition, televising, Internet posting, or other publication of my Posted Story and Personal Images, irrespective of whether a fee for its use is charged by any third party.

I declare that I am eighteen (18) years old or older and am legally competent to agree to these Terms & Conditions. I understand that the terms herein are contractual and not a mere recital, and that these Terms & Conditions are legally binding on me. There is no time limit on the validity of this agreement nor is there any geographic limitation on where these materials may be distributed. These Terms & Conditions are governed by the laws of the Commonwealth of Pennsylvania, and any claims arising hereunder shall be brought exclusively in the state courts located in Chester County, Pennsylvania.
